Home Care Services

Our team helps families manage the day-to-day responsibilities of caring for a loved one who may be home-bound and unable to perform all activities of daily living or ADL’s.  Unlike home health, home care needs are classified as personal care and can be as simple as needing one or more of the services listed below.

Services provided by home care aides may include:

  • Meal preparation
  • Light Housekeeping
  • Help dressing, bathing and grooming
  • Accompany to medical appointments
  • Reminders to take medicine
  • Grocery shopping and errands
  • many other “non-clinical” home care services are available and can be customized to your needs

Nursing & Therapy Services

Our skilled nursing services are provided by registered nurses. We work side-by-side with your doctor to coordinate the best in-home care.  Many utilize our nursing and therapy services following a hospitalization, when there has been a decline in functioning, or following a stay at a skilled nursing facility – to help the senior transition back to independence. Skilled nursing is a term that refers to a patient’s need for care or treatment that can only be done by licensed nurses and therapists. Home health care is usually less expensive, more convenient, and just as effective as care you get in a hospital or skilled nursing facility.

Our team can quickly assess your needs and dispatch a qualified registered nurse to your home within 24 hours for an assessment. We also take this time to meet with the family and develop a comprehensive care plan. Skilled services require a doctor’s order and  we are happy to contact their office and obtain all the necessary details.

Skilled home health care services can include:

  • Physical therapy
  • Occupational therapy
  • Speech therapy
  • Administration of medications, including injections
  • Diabetic care & monitoring insulin
  • Respite care
  • Medical tests
  • Monitoring of health status

Payer Sources

Home care services are generally paid for “out-of-pocket” unless you have long term care insurance, in which case you should check your policy to determine specifics. Medicaid can cover these services for qualified individuals.

Since it is considered medical care, skilled home health is covered by medical insurance, including Medicare and Medicaid.

Support When You Need It

Home health and home care are not exclusive services. Many families find that utilizing these services together is best to help a senior who, for example, is recovering after a hospitalization. The home health staff address clinical and rehabilitative needs during the transition home, while a home care aide can help with personal caregiving and household chores that the senior requires assistance with during his or her recovery.

There is no commitment as to how much you need to use our services. They are at your beck and call as you need them, whether it be for an hour a day, once a week or 24/7. Together with your physician and family members, we’ll determine the best care plan to meet your needs.
